We accept most PPO plans and many HMO plans, and we want to make the billing process as smooth as possible for your family. Please take a moment to review the information below before your child’s visit so there are no surprises.
Not sure if we’re in your network? Just let us know when you’re booking and we’ll be happy to confirm that we accept your insurance and verify your coverage before your visit.
What to Bring to Your Child’s Appointment
- Your child’s insurance card — This is required for new patients or any time your insurance has changed since your last visit. If you haven’t received your new card yet, bring as much of the following as you can: insurance company name, claim address, copay amount, policy holder name, policy number, group number, and suffix.
- Your copay (if applicable) — Payment is due at the time of service.
- Your child’s immunization record — Especially important for new patients
- Relevant medical records — For children with complex or longstanding medical histories, please bring any medical, school, or other records that help paint a fuller picture of your child’s care.
- Knowledge of your coverage — As a courtesy, we’ll bill your insurance directly, but you are responsible for any remaining balance. Every plan is a little different, so we recommend contacting your insurance company if you have questions about what’s covered.
Please note: Being on our accepted insurance list means we can submit a claim on your child’s behalf. It does not guarantee that your insurance company will cover all or part of the services provided.
PPO Plans
We are participating primary care physicians for most major PPO plans. Because coverage can vary based on your specific plan and policy type, we recommend calling your insurance company to verify your particular benefits before your visit.
Below are some of the PPO plans we currently accept:
- Aetna PPO
- Anthem Blue Cross PPO
- Blue Shield PPO
- Carpenters Health and Welfare Plan
- Cigna PPO
- Health Net PPO
- Motion Picture
- Operating Engineers
- Preferred Health Network (PHN)
- Screen Actors Guild
- Southern California Pipe Trades
- Southwest Administrators

HMO Plans: Important Steps Before Your Visit
If your child is on an HMO plan, please double-check the following before scheduling:
- Primary Care Physician (PCP): Your child’s listed PCP needs to be one of our Pomona pediatricians:
- IPA / Medical Group: Your child’s plan must be associated with one of the following:
- Pomona Valley Medical Group
- Atlas Healthcare Physicians IPA
Medi-Cal / IEHP
If your child is covered through Medi-Cal, we’re happily accepting new patients through:
- LA County Medi-Cal: Blue Cross Medi-Cal – Atlas Healthcare Physicians IPA or Access IPA
- Health Net Medi-Cal: Atlas Healthcare Physicians IPA or Prospect Medical Los Angeles IPA
- San Bernardino Medi-Cal: IEHP Direct
Not Sure If We Accept Your Plan?
If we don’t have an agreement with your insurance company or if your child does not have health insurance, your account would be set up as self-pay, with payment expected at the time of service.
If you’re on a PPO, we participate with most major plans, but coverage can vary. We recommend checking with your insurance provider before your visit to be sure.
Still not sure? Give our office a call at (909) 629-5067 and we’ll be glad to help you figure out the best path forward.
