Commercial HMO plans
We accept many HMO plans. The following plans must have one of our doctors chosen as a primary care physician and the selected medical group as Pomona Valley Medical Group or Allied Pacific of CA IPA:

Medi-Cal Plans
We accept several Medi-Cal plans with various medical groups. Select a plan with one of our doctors as a primary care physician from one of the following medical groups.
